We create the test case in Test Manager and then pull up the work item number in Test Steps Editor to add the steps. Can't wait 'til it's a plug-in. I think 'WorkItemIdMRU' is not getting set properly, You have an error in LoadMruFromRegistry method on second run.

Please let me know if I am missing something.
